Two volcanoes rising from the middle of a freshwater lake, connected by a narrow isthmus. Ometepe looks impossible on a map and even stranger in person. Concepcion is the active cone (1,610m), Maderas is the dormant one with a cloud forest and crater lake on top. Between them: farms, small villages, howler monkeys, and roads that test every scooter suspension. The island moves slowly, internet is patchy, and the main currency is time. Give it at least three days or you will feel rushed.

The ferry from San Jorge (near Rivas) crosses to Moyogalpa in about an hour and costs under 100 NIO. No advance ticket needed, just show up at the dock. Departures run roughly hourly during the day, fewer on Sundays. From Managua or Granada, bus to Rivas first (2-3 hours), then a short taxi or bus to San Jorge port. The lake crossing can get choppy in afternoon winds, so morning ferries are smoother.
Dry season (November through April) keeps roads passable and volcano trails less muddy. January and February balance good weather with moderate visitor numbers. The Concepcion hike should only be attempted in clear conditions, as cloud cover above 1,200m makes navigation dangerous. Rainy season turns interior roads to mud and makes the Maderas summit trail genuinely treacherous.
ATMs exist in Moyogalpa but can run out of cash. Bring backup from the mainland. Internet is weak across the island, not reliable for video calls or remote work. Scooter rental is practically essential because distances between spots are longer than expected and roads are rough (loose gravel, steep sections). Rent from Moyogalpa on arrival and fuel up there too, stations elsewhere are sparse.
Ometepe is one of the safest spots in Nicaragua. Crime against tourists is rare. The real risks are road-related: loose gravel, wandering livestock, and scooter inexperience cause most injuries on the island. Wear closed shoes and long pants if riding. The Concepcion volcano hike is strenuous and demands a guide, people get lost or injured on that trail yearly.
